THIS WEBSITE AND OUR SERVICES DO NOT PROVIDE EMERGENCY CARE. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO THE NEAREST EMERGENCY ROOM IMMEDIATELY.

2. Eligibility

You must be at least 18 years old and legally able to enter into a binding agreement to use this website and request services. Clinical services are only available to eligible patients located in Arizona at the time services are rendered, unless otherwise expressly stated.

3. Informational Website; No Guaranteed Treatment

Content on this website is provided for general informational and marketing purposes only and does not establish a physician-patient relationship by itself. Submission of a form, payment of a consultation fee, or use of the website does not guarantee that treatment, a prescription, ongoing care, or any specific outcome will be provided. 4. Consultation Requests and Clinical Services

Our practice offers physician-guided telemedicine and wellness-related services.
